No. You must send us a letter requesting a duplicate license and appropriate fee ($6). Your letter must have your name as it appears on your license, your driver’s license number or social security number, your date of birth, a telephone number or email address, your signature and an address where we can forward your license to.

Yes. Your daughter may come in to take the written test. She must present a certified copy of her birth certificate, social security card and divorce decree showing custody. In addition, if your name has changed, you will need to present name change documents (divorce decree, marriage certificate). We will complete an application card for you to send to her father for his signature. Father must sign in front of a Notary Public. The written test will be held for 30 days.

You are receiving this letter because of your recent involvement in a motor vehicle traffic accident. If you had insurance on the vehicle you were operating at the time of the accident, contact your insurance company and inform them that you are required to submit the Form SR-21. Be prepared to provide your insurance company with a copy of the letter. We must receive the Form SR-21 before the “order effective date” located on the bottom of the letter. The Form SR-21 is verification of insurance. Even though you showed the Police Officer your insurance card at the time of the accident we are required by law to obtain verification. If you did not have insurance at the time of the accident, you must comply by providing a Security Deposit as stated on the letter, obtain a Release from Liability from the other party whose property was damaged, or provide evidence of an installment agreement that you have entered into. If you are unable to do any of the five items, your license will be suspended for a period of two years.

General Road Test Information

Please ensure the vehicle you are using is ready for the road test. You may use the following list as a guide.

  • The passenger compartment, seat and floor space must be free of any obstructions.
  • The rear-view mirror must be free of any hanging materials.
  • All turn signals work properly.
  • All brake lamps are properly working.
  • Brake & turn signal lamp lens are not cracked or broken.
  • Both front & rear license plates are securely mounted.
  • License plates must be entirely unobscured and be kept reasonably clean. No covers.
  • Vehicle’s horn is audible and in good working condition.
  • All lug nuts are in place and tire tread depths are at least 2/32 of an inch.
  • Vehicle is free of any obnoxious odors and/or parasites and insects.
  • Vehicle should be reasonably clean.
  • Vehicle instrument panel gauges should be working properly and NO warning indicator lights should be on.
  • Driver and passenger seat belts are clean and in good working condition.
  • Door handles, windows and door locks are in good working condition.
  • Weight tax decal and Safety check decal are on vehicle.
  • Windshield
    • No cracks or chips.
    • Stickers or other nontransparent material are allowed only at the lower right hand corner of the windshield and no more than four inches by six inches in size.
  • Side & Rear Windows:
    • Stickers or other non transparent material are allowed in an area that DOES NOT obstruct the driver’s clear view of the highway or any intersecting highway.
  • Window Tinting
    • Approved by an official safety inspection station and noted on the vehicle’s safety check inspection certificate.
    • Tinted windows that appear too dark are subject to tint meter verification to ensure compliance prior to the road test.
    • Front windshield shade tint must be above the AS-1 marker or no more than four (4) inches from the top center of the windshield, if there is no AS-1 marker.
    • For convertible and Jeep-type vehicles, the canopy top must be engaged in the closed and locked position covering the passenger compartment.

All road tests are subject to cancellation due to inclement weather or unsafe road conditions as determined by the driver license examiner.

Mandatory waiting period for road tests
  • New Drivers –18 yrs & older: 2 week waiting period
  • Under Age 18: 180 days waiting period. Driver’s Education required*
  • If you are under the age of 18 you must provide the Driver Education and Behind the Wheel Certificates and instructor number at time of appointment

You are responsible for providing a vehicle that will be used for road test. Rental vehicles may not be used.

Your vehicle should be reasonably clean and free of insects, animal hair, and obnoxious odors.

If you possess a foreign driver’s license and it is not in English, a passport and an international driver’s permit is also required.

You must check in 30 minutes prior to your scheduled appointment accompanied by a licensed driver, 21 years old or older. Failure to check in on time may result in the rescheduling of your appointment.

The following must be presented:

  • Valid Learner’s Permit
  • Licensed driver with valid driver’s license(21 years or older)
  • If under 18 years old, Driver’s Education and Behind the Wheel Certificates.
  • Valid County Certificate of Registration
  • Valid Safety Inspection Certificate
  • Hawaii Motor Vehicle Insurance Card
  • **Vehicle must have ¼ tank of gas.

Valid, original vehicle documents (faxed, photo or e-copies WILL NOT be accepted).

Verify all the information on the Hawaii Motor Vehicle Insurance card and Certificate of Registration match.

  1. Make and type of vehicle
  2. Vehicle Identification number
  3. Names of the insured and insurer, policy number and effective dates of coverage.

If there are two (2) registered owners, the Hawaii Motor Vehicle Insurance card must show at least one of the names.

If the vehicle is leased, the Hawaii Motor Vehicle Insurance Card may be in the name of either the leasing company or the leasee.
